Dear Patient,
We would like to know how we can improve our services to you and how you perceive our surgery and staff.
To help us with this, we have set up a patient participation group so that you can have your say.
The group was established in 2009 and meets twice a year to discuss relevant practice issues and patient survey results. If you are interested in joining our patient group forum please complete the form below or ask at reception or telephone the practice.
The next meeting is 27th April at 6.30pm. New Members are always welcome so please come along and join us.
If you would like to join our ‘virtual patient group’ and are happy for us to contact you periodically by email for surveys and other such feedback based items, please fill out the form below and return it printed to the reception or post it in the ‘secure box’.
During the year we gain your feedback from a number of sources including the friends and family test, the patient survey and the patient participation group.
