Care Data

The care data system is being introduced so that information about you and the care you receive is shared, in a secure system, by healthcare support staff to support your treatment and care.

The NHS will use this information to plan and improve services for patients. They will link information from all the different places where you receive care, such as your GP, hospital and community service, to provide a full picture. This will allow them to compare the care you receive in one area against the care you received in another, so they can see what worked best.

YOUR CHOICE
You have the right to prevent confidential information about you being shared or used for any purpose other than providing your care, except in special circumstances. If you do not want your information to be shared with the Care Data system please let us know so that we can make a note on your medical record. This will prevent your confidential information being used other than where necessary by law, (for example if there is a public health emergency)

Your choice will not affect the care that you receive.

A new animated video has been developed by NHS England and the Health and Social Care Information. The video,  “Better Information Means Better Care”, accompanies a leaflet Better information means better care that is being delivered to every house in England.

The patient information line is also there to answer any questions or concerns that people have about how their data is used for these purposes. The information line is open Monday-Friday, 8am-8pm and on Saturdays 9am-3pm. The number to call is 0300 456 3531. A text phone service is also available on 0208 7428620.

You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have concerns or wish to prevent this from happening, please speak to practice staff or download the Care Data Opt Out Form, complete it and return it to the practice.
